Prospective
Students
Admission decisions
are made without regard to disabilities.
All prospective students, regardless
of a disability, are expected to present
academic credentials at or above the
minimum standards for admission as
established by the Admission Committee.

Requests
for applications for admission to
CC should be directed to the:

After acceptance to CC, students
with disabilities who will be requesting
reasonable accommodations should
contact the Office of Disability
Services (contact info above) to
begin the process of establishing
eligibility for services.
Once
Admitted to Champlain College
CC is committed to insuring that
students with disabilities have an

The Office of Disability Services
(ODS) is available to assist you in
identifying your special needs, and
providing and coordinating accommodations,
support services and programs that
will give you an equal opportunity
and help you maximize your learning
potential as you make the transition
into, through, and out of the college.
Once you have chosen CC, the following
guidelines will assist you in making
a smooth transition into the college:
- Review the Office of Disability
Services web page for accommodations,
services, policies and procedures.
- Contact ODS at 802-865-5484 or 802-651-5961,
to initiate services.
- Submit current, comprehensive,
professional documentation of your
disability to ODS.
- Schedule an appointment to register
with to discuss your documentation,
appropriate accommodations, services,
and the impact your disability may
have on course selection and scheduling.
PLEASE NOTE:
It is the student's responsibility
to arrange for certain services which
are outside the scope of the Office
of Disability Services and the College.
These services include, but may not
be limited to: attendant care, mobility
training, items / services of a personal
nature and sources of financial aid.

you regarding your special needs and
possible local resources
Who
Is Eligible?
Disability
Services are available to all currently
enrolled students who have a documented
disability that substantially limits
them in one or more of life's major
activities.
Individuals eligible for services
include, but are not limited to students
with Mobility, Orthopedic, Hearing,
Visual, Speech, and Learning Disabilities
What
documentation do I need?
Confidentiality
Statement
All contact
information and documentation received
in Disability Services is kept in
separate confidential files within
the office. No information concerning
inquiries about accommodations or
documentation will be released without
written consent by the student.
In order
to fully evaluate requests for accommodations
or auxiliary aids, CC’s Office
of Disability Services will need documentation
of the disability that consists of
an evaluation by an appropriate professional
that describes the current impact
of the disability as it relates to
the accommodation(s) requested.
Documentation
provided will be used by the Office
of Disability Services to evaluate
requests for accommodations or auxiliary
aids. The evaluation process includes
a review of the documentation itself
and in the context of documentation,
on the fundamental goals and essential
standards of the program, course,
service, or benefit in question.
The evaluation
process will generate a list of potentially
reasonable accommodations that will
then be reviewed based on potential
effectiveness, preferences of the
requester, maximum level of integration,
and the potential for an undue financial
or administrative burden.
Did you have an IEP or 504
Plan in high school?
For individuals
who have recently been receiving services
from a public school system; the information
requested would most likely be contained
in the Psycho-Educational Evaluation
report from your most recent Triennial
Review. Some of the information may
also be contained in an IEP, 504 Plan,
or Transition Plan. You may wish to
include this with your Psycho-Educational
Evaluation report as supportive documentation.
You must request this information
separately from your high school transcripts
and should make sure that it is received
by the Office of Disability Services.
Are you
transferring from another college
or university?
For individuals
transferring from another college
or university; information related
to your disability will not be sent
with a transcript request. You must
request that information be sent to
CC’s Office of Disability Services
separately. Additionally, the information
CC’s is requesting may or may
not have been a part of your previous
college's evaluation process. If you

How
do I register for services & Request
Accommodations?
Eligibility must
first be determined in order to make
arrangements for using services and
accommodations from ODS. Providing
current documentation to the Office
of Disability Services that establishes
a disability is the responsibility
of the student.
INCOMING
FRESHMEN: Contact the Office
of Disability Services after acceptance
to CC in order to determine what documentation
is necessary to determine eligibility
for services.
- Register for services
- Identify special needs
- Discuss the impact of the disability
on course selection and scheduling
- Create an Access Plan for the
upcoming semester
CURRENT
STUDENTS: Please contact
Disability Services prior to, or as
early in the semester as possible
and provide appropriate documentation
of your disability. After appropriate
documentation is received, student
information will be gathered and confidentiality
will be discussed. The Director will
then determine, with the student's
input, what services and accommodations
are needed. Accommodations are based
on the currency, depth and breadth
of documentation, along with the student's
input

Tips for

Housing Accommodation Request Form
Champlain College students with documented disabilities may request special housing accommodations within the context of current housing policies.
Housing accommodations forms need to be returned by the following dates:
- Incoming Students: June 15th
- Returning Students: By Housing Deposit Deadline (usually mid-March)
Forms not received by deadlines will be accepted and considered, but there is no guarantee that late requests will be met.
