The student’s provider (a certified treatment professional) must complete and return the Housing Accommodations Documentation Request Form, which is a downloadable PDF.

Returning students: November 15
Students must coordinate the submission of the forms noted above. Once received, the Office of Accessibility will review the information and contact the student if further information is needed.
Forms not received by the deadline will be accepted and considered, but there is no guarantee that late requests will be met.
The Office of Accessibility offers a wide range of services to ensure students with disabilities have equitable access to their education.
Test accommodations are any modifications made to tests or testing conditions that allow qualified students to demonstrate their knowledge and skills in a testing environment.
Transportation accommodations include a shuttle service to ease mobility to and from class, as well as designated parking spaces on campus for those who have a temporary or state disability placard. A Champlain parking permit is also required.
Champlain’s on-campus housing includes Victorian-era mansions and contemporary-style residence halls. Check them out!
