Concentra is Champlain College’s preferred workers’ compensation medical provider. Concentra offers services in five main areas: occupational health, physical therapy, health and wellness, urgent care, and primary care.

If a work site injury occurs, employees should immediately notify the supervisor of their injury and the supervisor and/or employee must complete the Employee Injury/Incident Report.

Procedures for Work-Related Injuries

2. Non-emergency

  • Either area walk-in clinics that specialize in work-related injuries (non-emergency). No appointment is needed. Supervisor and/or Employee must complete the Champlain College Injury Report.
  • After the initial visit at ClearChoice or Concentra, an employee may go to his or her own medical care provider for follow-up care or continue with ClearChoice MD/Concentra.

Reporting and follow-up

    • Complete the Employee Injury/Incident Report and forward to your supervisor and the People Center.
    • Provide the supervisor and Debra Sailer Dayman (the People Center) with information about the incident and injury within 24 hours so it can be reported to insurance and the State of Vermont. If Debra is unavailable, report to Hannah Puryear (People Center) or Ted Winokur (Risk Management)
    • Follow up with the supervisor, doctor and The People Center about return to work or arrangements for light duty work. A doctor’s note is required for missed work/reduced work for a work-related injury, or to return to regular work.
    • Respond to Travelers Insurance Co. when they contact you to get more details about the incident and injury.
    • Ensure the Champlain College Employee Injury/Incident Report is completed and sent it to Debra within 72 hours, if appropriate. Form can be found on the People Center website under Benefits – Workers Compensation.
    • Follow up with the employee and the People Center about return to work or arrangements for light duty work. A doctor’s note is required for missed work/reduced work for a work-related injury, or to return to regular work.
    • Report incident to Travelers. Travelers files First Report of Injury to State of Vermont.
    • Follow up with supervisor and Risk Management on safety and prevention issues and accident investigation as appropriate.
    • Receive reports from ClearChoice MD and/or Concentra and the People Center about injured employees.
    • Follow up with the People Center about health and safety issues and remediation.
